900 ONLINE Blockage or Unblockage Form
REQUIREMENTS:
Corresponding form to be completed and signed on behalf of the account owner or legal representative if the account belongs to a corporation.
Copy of the telephone bill of the line to be blocked or unblocked.
Copy of the ID card, passport or residence card of telephone line’s owner.
Copy of legal capacity (with no more than one month of issuance) and of legal representative’s ID card if the telephone line is to the name of a company.
