- Corresponding form to be completed and signed on behalf of the
account owner or legal representative if the account belongs to a
corporation.
- Copy of legal capacity (with no more than one month of issuance)
and of legal representative’s ID card.
- If
you wish to authorize a third party to receive the envelope with
the access password, please include his/her complete name and ID
card in the form’s Comments field.
- Time: 24 Calendar Hours.
