REQUIREMENTS:
Corresponding form to be completed and signed on behalf of the account
owner or legal representative if the account belongs to a corporation.
Copy of legal capacity (with no more than one month of issuance) and of legal representative’s ID card.
If you wish to authorize a third party to receive the envelope with the access password, please include his/her complete name and ID card in the form’s Comments field.
